Jay Elementary’s Kindergarten Registration

Jay Elementary School will be having their Kindergarten registration on Thursday, May 2nd from 8:00 am until 6:00 pm.
Your child must be 5 years old on or before Sept. 1st 2013 in order to start kindergarten next year.

Please bring the following items with you to registration:
Child’s birth certificate
Florida Immunization card (shots must be up to date)
Up to date Florida physical (must be performed within 1 year from enrollment date- 8/20/13)
Social security card/number
Proof of residency (utility bill or rental agreement… Driver’s license is NOT acceptable)
Custody papers (if applicable)

All items MUST be turned in before school begins on August 20, 2013 in order for your child to be enrolled.
