JHS Sports Physicals -Get Your Forms NOW
Jay High School Athletic physicals will be given on May 30th starting at 8:00 am at the Jay Medical Center. The school will be taking students there by bus by grade level. Student Athletes must pick up a physical form in the office and have it fill out and notarized before they can take a FREE physical.
Jay Medical Clinic has been hosting the Royals’ annual sports physical for many years. The Doctors: Dr. C.David Smith, Dr. Jeff Kelley, Dr. Michael Smith and Dr. Marion Stewart, donate their time and facility for the players.
